Pre-Camp Checklist
The following online forms and records are required and should be uploaded to CampInTouch in advance of your camp session:​

Immunization Record: must be provided by doctor’s office and include all immunization requirements for the grade they will enter in the upcoming school year

Health History Form: fill out info about your camper’s health
- Registration Date: Deposit of $100/session (nonrefundable)
- March 2: 50% of total tuition due (nonrefundable)
- May 1: Remaining balance due (nonrefundable)
Please note that missed payment deadlines may result in cancellation of registration and forfeiture of fees paid.

Below is a list of items that your camper may need to bring to camp. Please ensure all clothing is appropriate for being active at camp.
Please label all items with your campers’ name.
Crossroads cannot be held responsible for any valuables brought to camp.

To ensure the safety of all campers and staff, the following items are not permitted at any of Crossroads' summer or year-round programs: guns & weapons, illegal substances, marijuana/tobacco-related products, alcohol, smoking-related items (including e-cigarettes, vape pens, etc), fireworks, animals, personal electronic devices (such as cellphones, tablets, laptops, etc), personal sports equipment, and medication (unless checked in with our health center).
